How We Work
We are able to accept referrals from Case Managers, Continuing Healthcare Teams (CHC), Clinical Commissioning Groups (CCGS), Solicitors and directly from clients and families.
When Dunedin Complex Care receives a referral we would look to carry out an initial clinical assessment, as part of this assessment, carried out by our nursing team, we will not only be considering the clinical requirements of your support, but also they type of team you wish to have. We can train clinical skills but find that having the right attitude, personality and approach is far more important to having a long lasting, therapeutic relationship.
Following a successful initial meeting we would then start to build the team. The client, family member and/or case manager is welcome to form part of the interview panel and write any specific questions they would like to be included in the decision making process. Our clients build the team. Dunedin Complex Care have complete empathy as to how intrusive it is to have people in your own home therefore this approach is fundamental to a long and sustainable package of support.
